What link should I put in the QR code?

Use a direct review link. For Google that is the search.google.com/local/writereview link tied to your Place ID. If you do not have one yet, make it with the Google Review Link Generator, then paste it here.

Where should I put the QR code?

Is the QR code free and does it expire?
